Welcome to The Valleys Primary Care Network which covers the semi-rural populations across the Holme Valley and Colne Valley area of Huddersfield working to deliver services to a population of around 54,000 patients.

Across the six member practices of Meltham Group Practice, Slaithwaite Health Centre, Oaklands Health Centre, Elmwood Family Doctors-Meltham, Colne Valley Group Practice and Honley Surgery we have practices which are rated ‘Outstanding’ by CQC and are actively involved in the training of the primary care workforce.

As a group we work together to improve the healthcare of our local population together with our local partners within the NHS and voluntary sector.
