Clinical Director - Dr Dil Ashraf

Clinical Director for the Valleys PCN and Partner at Meltham Group Practice for 17 years. I also served on the Greater Huddersfield CCG board for 6 years. I am committed bring the Valleys Practices together with Local Partners to help our local population. I enjoy keeping fit and trying to keep up with my children.

As The Valleys PCN Manager, my role is to support the PCN to develop and implement service improvements.  By working together with local partners, we can ensure we offer more coordinated, personalised and integrated health and social care to our local population. Outside of work, I enjoy spending time with my daughter, walks with our cockapoo and seeing friends and family!

I am a Trainee Psychological Wellbeing Practitioner and I work for South Kirklees IAPT Service. IAPT stands for Improving Access to Psychological Therapies and my aim to help people get quick and easy access to the best type of therapy for their individual needs. I can teach you skills and techniques you can use to help manage everyday ups and downs. Our service which is available across The Valleys Primary Care Network can offer a range of treatment options which can be delivered in a variety of ways such as over the phone via video call or internet-based therapy and courses.

A First Contact Practitioner for The Valleys Primary Care Network Physio First service. I qualified as a Physiotherapist in 2008 at Coventry University and have since worked in both the NHS and Private sector.

Care Coordination is there to support patients through their journey of care, assist the practices in securing appointments for patients with specific care needs and offering assistance when needed to patients. Outside of work, I like to socialise with friends and take long walks with my dog.

I will listen to what matters to you and have regular coaching conversations with you to help improve how you feel, build confidence, and overcome barriers to you living independently.

I am a Physician Associate working for The Valleys PCN. The PA role is relatively new in the NHS, we are medically trained clinicians that work alongside doctors and other healthcare professionals to aid in the diagnosis and management of patients. My role involves working as a generalist clinician, this means I can provide care for patients with acute illnesses as well as those dealing with long-term health conditions. I have a keen interest in developing my knowledge and skill set further in the management of diabetes and hope to offer a service accessible to all patients across the PCN. I completed my postgraduate degree in PA studies and the University of Leeds and qualified at the end of last year, I look forward to developing my career in primary care.

I am a Clinical Pharmacist and I work across The Valleys Primary Care Network. I qualified as a Pharmacist from Bradford University in 2010 and have since worked in community pharmacy. I have started working in primary care since February 2020 and I am thoroughly enjoying it. Outside of work I am a mum to two young boys and enjoy spending time with my family and visiting sites of interest.
